Before a coach can work with students, you must have your Pupil Activity Permit from the ODE up-to-date AND on file with the Worthington Schools HR Office.

1. FBI Fingerprint Background Check - Worthington coaches must have an updated BCI and an FBI background check every five years.
- Background checks can take up to 30-60 days, which is why it's important to start the process right away!
- Worthington Schools will process a FBI background check for you at a cost of $26. Payment can be made with a check or money order or debit/credit card. We do not accept cash payments.

- If you already have a FBI background check within the last year that's on file with ODE, click here to see if you can use it for this process.
2. PAID COACHES – IMPORTANT!
If this is your first year as a paid coach – by Worthington District or the Teams Booster Group – you will need to print and complete the entire payroll packet. Please call your HR coaching contact to schedule a time to bring these into the office.Remember - In addition to the completed Payroll Packet, you must bring your Driver License, Social Security Card and a Voided Check or information from your bank for the Direct Deposit account. If you are a returning coach and your direct deposit account has changed, you will need to submit a new direct deposit form and voided check or information from your bank. Your HR contact can help you with that process.
3. PAP License - An up-to-date Pupil Activity Permit from the Ohio Department of Education is required for any student activity program that involves athletics, regular physical activity or activities with health and safety considerations. It is good for a three year period from June 1 until July 30. You do not have to retake the Fundamentals of Coaching Class to renew the PAP. (Click here to see the PAP ODE Application Instructions.) You do need the following:
- Sports Safety Training Seminar - This class is taken each time you renew your PAP. It can be taken online or onsite. Click here to find a class.
- CPR Certification - Your CPR must be up to day. If yours is expired, you will need to retake the class. Returning coaches can take this class in person or online. Click here to see a list of class options.
- Concussion in Sports Certificate - Concussion training must be updated when your PAP is renewed. This certificate can be obtained free through an online class. Click here to complete the class.
- Sudden Cardiac Arrest - After Ohio Senate Bill 252 (Lindsay’s Law) was passed in 2017, coaches for youth are required to complete this Sudden Cardiac Arrest training annually. Click here for additional information.
3. Submit all of the certificates from the above classes to your contact in the Worthington Schools Human Resources Department.
4. Log into your OH|ID Portal Account with the Ohio Department fo Education (formerly known as the SAFE Account)
NOTE: You will need the Worthington City School Information Retrieval Number (IRN) during this process. It is 045138.
5. Once all of your certificates are on file with HR, Worthington Schools will provide the superintendent's signature on your ODE PAP application. You will receive an email from ODE when the Worthington e-signer has approved your application. ODE will review your application and email you when the PAP license is ISSUED.
IMPORTANT: Your Athletic Director will be notified when you are clear to coach. You are not permitted to work with students or teams until the above certificates are on file with HR and the ODE has ISSUED your new Pupil Activity Permit.
