State of Ohio Immunization Requirements for Seniors

My name is Julie Frank, and I am the new school nurse at Thomas Worthington High School. I have worked in Worthington Schools for over 20 years and have had 4 children graduate from Thomas Worthington High School. My favorite part of being a School Nurse is getting to know the students and their families. I wanted to remind you that the Meningococcal vaccine is still a State of Ohio immunization requirement for all students entering 12th grade. The Centers for Disease Control and Ohio Department of Health both recommend keeping children on a normal schedule of vaccines during the COVID 19 pandemic.
Please call your physician and seek advice to safely schedule an appointment to have your child receive the required Meningococcal vaccine. You may also schedule an appointment with the Columbus Public Health Immunization Clinic by calling 614-645-8180 or clicking HERE. Franklin County Public Health also offers Immunization Clinics; please call 614-525-3719 to make an appointment.
Once your child has received the vaccine, please provide TWHS with a copy of the immunization record. You may email the proof of vaccination to Nurse Julie Frank at email@example.com or fax a copy to 614-450-6391.
