Transition Services

Sheltered Internship - Internship - Enclave
Worthington Schools offers several courses for students with disabilities who are working on pre-employment skills.  Students complete unpaid work experiences at a school or community based worksite with the support of an Intervention Specialist or a Transition/Job Coach.

Job Training
This course designed for students with disabilities who need assistance in seeking, obtaining, and maintaining competitive employment in the community.  Students be at least 16 years of age and must have the ability to transport self or to obtain own transportation. 

Placement in a transition course is an IEP team decision.  Please contact us if you would like more information about one of Worthington’s transition courses. 

Delaware Area Career Center
Information about Delaware Area Career Center programs can be found on their website and in the counseling center at your son/daughter’s high school.  Any student with an Individualized Education Plan that attends DACC will have access to the Vocational Special Education Coordinator (VOSE) at DACC. 

Transition Night:  Community Resource Fair

Transition Night is a resource fair for families of students with disabilities to learn about agency supports, community resources, and post-school options.  Transition Night is held annually in the fall. A resource guide from last year’s transition night can be found at this link.

College Night

College Seminar for students with an Individualized Education Plan or 504 Plan is held annually in fall.  This event is for students and families in area high schools with a focus on how to apply for and access accommodations at the college level.  A panel of representatives from many different colleges will be available to answer questions.

Worthington Schools partners with several transition programs.  In order to be considered, a student must be recommended by their IEP team and be accepted into a program through an application process.  Students attending a transition program must have fulfilled all of their academic requirements for graduation and defer graduation in order to continue to receive transition services.  If you would like more information about one of these programs, please contact the district Transition Specialist. 

Campus Based Transition Programs

The ESC of Central Ohio has partnered with area institutions of higher education to give Central Ohio students with disabilities an opportunity to explore, develop and refine skills necessary to enter competitive employment. Students focus on individualized transition plans to achieve future adult living and career aspirations.

The ESC offers three unique programs on the campuses of Columbus State Community College and The Ohio State University, which serve students 18-22 who have completed or met all graduation requirements, and have elected to defer their high school diploma. Each applicant for CBT is considered a candidate for all three programs and is accepted based upon availability, individual needs and work experience.

Franklin County Board of Developmental Disabilities
The Franklin County Board of Developmental Disabilities (FCBDD) is a county agency providing supports to children and adults who have developmental disabilities. Services are provided to residents of Franklin County, Ohio.  Additional information and an application for services can be found on their website.

Opportunities for Ohioans with Disabilities
OOD helps students with disabilities successfully transition from school to work. Transition teams work together to develop activities and experiences designed to provide students with the necessary skills and knowledge to reach their post-secondary goals.  OOD’s goal for successful transition includes planning for, finding and keeping a job prior to exit from high school.  After employment counselors conduct follow-up activities to make sure employment is sustainable. Transition from school to work information.

Contact the Transition Specialist or Job Training Consultant for assistance with referring your son/daughter.  An online application for services can also be found on the website.

Social Security Administration
The Supplemental Security Income (SSI) program makes cash assistance payments to eligible aged, blind and disabled individuals (including children under age 18) who have limited income and resources. Individuals may apply for benefits and find additional information on their website.

State Support Team Region 11
State Support Team Region 11 promotes strong family/educator partnerships to enhance the school success for children with disabilities.
