For currently-enrolled Worthington Students - This form is used to notify the district of intent to enroll a student in a Worthington School other than the one to which the student is assigned.
Note: You still must live within the Worthington School District boundaries to take advantage of intra-district open enrollment
This form serves as a guide for what immunizations are needed to attend school. A form provided by your physician's office can also be used.
