Enrollment - What you need to know before you start!

1. Your family MUST live within the Worthington School District boundaries to enroll! You can find more information in the School Board Policy.
    
2. The enrollment process is now online, and all documents will be uploaded directly to the Welcome Center. 
    
3. We are working to process the enrollments as quickly as possible.
    
4. Make sure your uploaded documents are readable and printable. We recommend uploading in a PDF format.
   If documents are missing, unreadable or cannot be opened, you will receive an email from the Welcome Center with details of what needs to be completed for your child's enrollment. Poor quality documents will delay the enrollment process.
5. During the registration process, you will need to provide addresses, phone numbers and emails for parents and guardians. Additionally, phone numbers are required for all emergency contacts that are listed. No more than three (3) emergency contacts per student are necessary. 
    
6. Custody documents are required by the state of Ohio. You should be prepared to upload your entire court filed and stamped shared parenting plan or divorce decree showing which parent is identified as the "residential parent for school placement purposes."
    
7. You will need the following documents to enroll:

• Student's Birth Certificate or Passport
• Parent/Guardian Driver's License, Photo ID or Passport
• Current Lease with signature pages or mortgage statement
• Current Utility Bill (dated within 30 days of enrollment)
• Student's Current Immunization Record
• Any Custody Documentation, if applicable
• Shared Residency Affidavits & Proof of Residency, if applicable

If documents are missing, this delays the enrollment process. Please be prepared to upload these documents.
